A 51-year-old male presented at the surgical outpatient department with pain in his left lower leg which he had had for several years. On physical examination a fascial defect on the anterolateral aspect of the lower leg was palpable approximately 12 cm above the lateral malleolus. Percussion over the fascial defect was very painful. Surgical exploration showed that at the fascial defect the superficial peroneal nerve perforated the crural fascia. The nerve was released from the fascia and a small fasciotomy performed. The symptoms disappeared directly after the operation. Entrapment of the superficial peroneal nerve is a relatively rare cause of pain in the lower leg. Specific to this diagnosis is a well localizable exquisitely tender point about 10-14 cm above the lateral malleolus on the anterolateral aspect of the lower leg. Decompression of this nerve with a small fasciotomy has excellent results.
